Problem/Suggestion: Within the Draft Death Certificate option, when entering box #27D information regarding a zip code for a resident outside of the U.S., the program prompts for the use of two dashes. When two dashes are entered, the entry is refused, and the message "-" NOT FOUND IN LIST FOR INFORMANT'S MAILING ZIP. ARE YOU SURE <N>? appears and if a "Y" is typed in, another message appears saying ZIP CODE MUST BE 5 DIGITS. Could the program be modified to accept the two dashes as an entry for a zip code address outside of the U.S.?

Evaluation: There is a difference in the Handbook between births and deaths for reporting zip codes for residents of foreign countries. The former (page 61) specifies five zeros (00000), while the latter (page 146) specifies a dash (-). The CDC specs state (page 37): "All zeros indicate residence is out of country." AVSS/EDR was programmed to be consistent with births and thus uses 00000 for foreign countries. This could be changed, but it will result in an inconsistency between births and deaths, or (preferably) the help for Field 27D of the DDC would be changed to instruct users to enter 00000.

Problem/Suggestion: Because, per state rules, "The coroner must complete the medical and health section data within three days after examining the body" and "The physician must complete the medical and health section data within 15 hours after death", I would like a warning message to appear if the dates signed by the doctor or coroner are more than eight days after the date of death.. This would help correct typing errors and alert the mortuary that the certificate will not be registered within the eight day limit given by the state. (This warning would appear for boxes #117 and #127.) If the dates in box #117 or box #127 are typed as being later than that date typed in box #39 (Date of Disposition), that entry should be refused, because the body can't be disposed of before the certificate is complete, including all needed signatures.

Problem/Suggestion: Under the hospital prompt is an attendant list that stores attendants and their related data such as medical license and address for automatic entry into the birth certificate at the hospital. AVSS does not prompt for the complete address as it does in other areas. This causes a problem because the street address is sometimes the only part of the address that is entered. OVR has rejected over 40 certificates in the last month because the city was not entered as part of the address on the certificate. I looked at all attendant lists for hospitals in Sacramento County and found 10 entries that did not have the city as part of the address. I found two entries that had a zip code, but no city. This represents a small portion of the total attendants, but creates the potential for many rejected certificates before the problem is found and corrected. Would it be possible to either prompt for the component parts of the address, or to place some type of explanation or warning that the address line is for the complete address including the city and not just the street.

Problem/Suggestion: The AVSS Missing File Number Report shows "No Missing Certificate Numbers" for a specified range, so I attempt a download of the records in the specified range. Sometimes, not all records in the range are downloaded, and are therefore considered Missing by my program. It appears that even though the record is on the AVSS system, it may not include it in the generation of the "New Recorder CBC File to Terminal (1400)" because one or more of the fields are missing data. At 9600 baud, we prefer NOT to have to re-download ANY records. However, when we download a range of records that does not contain every item, we will wait until the next day and try again. many times it has taken multiple downloads (of the same range) to get every record. After several unsuccessful attempts to get all records in our batch range, we will give up. This then requires us to delete the batch (on our IBM AS/400 System), break it down into multiple batches and re-enter it. In addition, we have to create a special batch for the missing record that will remain as outstanding until we can eventually get the data for it.
Request: Change the AVSS system to transmit ALL records that are not considered MISSING by the MISSING FILE NUMBER REPORT. Even skeleton records would cause far less problems for us than no record at all! We try to wait until there are no records missing in a given range (typically in multiples of 100 items) to avoid the added time of re-downloading or even worse, deleting our batches, breaking them down into sub-batches and re-entering them.
If the above change would cause problems with anyone, you could add another question into the user script of the download process such as: "INCLUDE INCOMPLETE RECORDS IN GENERATION? (Y)". This would allow this change to be OPTIONAL and if the user answered with a "n", the download would run just the way it does now.

Something like: "Are you entering a pending (P) or final (F) morbidity report?" In this example, if the user responds "P", then AVSS would go directly to DMR entry. If user responds "F", then AVSS would go directly to CMR entry (and check the entry for a link to a DMR and for a match to a CMR as is currently done).

According to the download file structure, Field 67, called UCOD (Underlying Cause of Death) is also missing. Since the variable UCOD is determined at the State level, we understand it may be blank. But, we cannot get any information regarding causes of death in the downloaded file. When we use the report generator #CDCLST3 and 4, we do get the listing of causes of death, 107a through D. However, it is difficult for us to relate these two files, since one of them is in a database format while the other is just the text format. It would be very useful for us to have the information about causes of death included in the download file.

This AMR was initiated as a result of a memorandum, dated 2/18/99, from Mr. Curt Lambdin of the Social Security Administration to Mr. Daniel Savino (Registrar, State of Connecticut). In his memorandum, Mr. Curt Lambdin informed Mr. Savino that EAB files are being sent in with combined years (i.e. 1998 and 1999 births). Section C2 B page 8 of the EAB 1998 contract states "States shall submit separate files for each calendar year and no more that one file per calendar year, per day."

Problem/Suggestion: In a recent memorandum, the Office of the Director of the Department of Health Services (DHS) reported that is will standardize the collection of race/ethnic (R/E) data by incorporating changes contained in Directive 15 from the Office of Management and Budget (OMB). In accordance with this memorandum, OVR will allow selection of up to three multiple races for an individual. The following seven principles should be considered when designing a means to collect, store, and code R/E data using AVSS:
1. The results should be compatible with DHS guidelines.
2. Printed values of R/E fields should be stored as AVSS data values (WYSIWYG).
3. The printed values must fit within the boxes on the Vital Records paper forms.
4. AVSS should continue to automatically code R/E without human intervention.
5. The coded values should accurately reflect the values reported by the self-declaration.
6. The manner in which AVSS collects and codes R/E should be documented.
7. Whenever possible, consistency with previous race coding should be maintained.

The following approach is suggested:
1 Increase the length of Fields 18 and 21 (Father's and Mother's race) by reducing the length of some of the fields on the same rows of the paper certificate. As shown in the table below for Father's race, Fields 20A-C would be reduced in length and Field 18 would be increase correspondingly. In the discussion below the same principles would apply to mother's race (Field 21) and the corresponding fields (22, 23A-C) on that row.

2. A new prompt, 18A, would be substituted for 18, and I18A would be coded accordingly. At the same time, 18 would be set equal to 18A and I18 to I18A.
3. There would be at least one additional prompt, 18B. If there was a response to that prompt, then I18B would be set, and a third prompt, 18C would exist with its corresponding I18C. Otherwise, race prompting would stop.
4. If more than one race were reported, the values would be separated by a "/" delimiter. This means that if three races were specified, the maximum length per race would be 8 characters unless one of the races was less that 8 characters.
5. Race values would be limited to a revised CBC list that will be edited to reduce the number of characters to no more that 12, or less whenever possible.
6. If the total number of characters for three races exceeded 24, AVSS would begin truncating the value to be printed for each race by one character, beginning with the last race, then successively truncate the second race, then the first, then iterating the last until only 24 characters remained. The untruncated values would still be stored electronically.
7. As discussed in Step 2, AVSS would continue to retain the old race codes for the first race listed. Using Father's Race as an example, 18 and I18 would remain as presently coded. This would maintain consistency over time for a large number of AVSS reports.
8. The values of UNKNOWN or WITHHELD could be specified only for the first race listed.
9. This programming would be set up in a 'time bomb' so that it would be automatically invoked on January 1, 2000 and would also apply to 1999 events that were registered on or after that date.

Evaluation: A significant programming and implementation effort. It will be important to coordinate the re-programming of AVSS with the re-design of the birth certificate form. for a successful implementation on 1/1/2000, the following conditions will be necessary:
1. A commitment from DHS to be responsible for producing and distributing the new forms to all AVSS hospitals no later than December 31, 1999.
2. In order to meet the schedule, DHS would have to give its commitment to this plan by the time of the AVSS-TAG meeting on April 26, 1999.
3. Test forms for revised birth certificate would have to be available no later than September 1, 1999.

When entering a CMR where the disease is Haemophilus Influenzae and either the date of birth or age is entered and it is not under age 30, the program should issue a warning so the record is not entered since instructions for reporting say "Haemophilus influenzae invasive disease means H. influenzae meningitis or infection of any other normally sterile body site regardless of Haemophilus serotype. Only cases under 30 years of age are reportable."

Problem/Suggestion: Requesting a Change when using the "home" password at the HCA. Change the prompted fields to include 13A. When a hospital creates a birth record for an "Ambulance" birth or a birth that occurs on the hospital property, but not in labor and delivery, it requires a typewriter to complete the birth certificate. Many times the ambulance attendant will not sign the birth certificate; hence item 13A will be "Unavailable" on the birth certificate. In this circumstance, the birth recorder has to print the birth certificate with a "-" in 13D, then type "Unavailable" in 13A on the birth certificate, and then go back into AVSS to edit 13D to "Unavailable" before transmitting it to the LCA.
Also, requesting a change at the LCA when creating a home birth that was an "unattended" delivery or an assisted delivery by a person "Unavailable" to sign in 13A such an ambulance driver. The same procedure as above must be done to prepare the birth certificate.
It would be more efficient if this entire procedure could be as one step done in AVSS before printing the birth certificate.

This file will be utilized by the State of Connecticut to generate the official Registration Report, and will be uploaded as a permanent birth record to the mainframe computer. The user shall be allowed to specify which drive and directory the data is to be written to. This new extract is similar to an existing option that generates a 1012 bytes layout under the LCT form.
Because Connecticut has based most current epidemiological analysis on the 1012 tape layout we would like to maintain the current layout and have the 1032 bytes layout as an adjunct.

Problem/Suggestion: Some reallocated records have bad census tract data. The LRD that registered the document has entered "dummy" census tract codes in the Census Tract fields. (They may arbitrarily assign a certain code to all Alameda County resident records to make their analysis easier.) We then have to allocate staff time to identify and clean up the "garbage" census tracts and replace them with real census tract data. This could be remedied by two methods: either don't transmit census tract data with the reallocates, or don't allow LRDs to enter unreal census tract data.

This AMR is in compliance with Social Security Administration memorandum dated April 9, 1999, requesting that a new 380 Logical Record Length (LRL) file be generated. New requirement increases the present record length from 342 characters to 380. There are no changes to the first 342 characters. The child's father and mother's social security numbers were added followed by fillers to the end of the present format. Change affects the Connecticut Central AVSS System only. None of the hospitals are affected.

Problem/Suggestion: Many times, when counties call OVR with edits, they need to have the edits updated on their system immediately. Currently there is no way to do that. Also, when we haven't connected to a county for reallocates, we have no way of testing the system to see if we are running into a time job problem, or a logon problem. The way we test it for receptions is to do a daytime startup, to see if we connect.
Suggestion: An option for a Daytime Reallocate Startup be added to the OVR system.

If you select "1" then MEXICAN will appear on the Birth Certificate. The next immediate question on the screen is Field (19) FATHER HISPANIC. If you type in "MEX", two choices appear on the screen: (1) MEX-AMERICAN (2) MEXICAN. So if in the normal course of processing birth certificates, one is rushed and automatically hits the "1" selection for FATHER HISPANIC (after all you hit the "1" when the question was FATHER'S RACE) you end up with the curious anomaly of a birth certificate showing the race for the father is Mexican but the Hispanic origin MEX-AMERICAN. Please have both number 1's & 2's match.

Problem/Suggestion: Currently the Mother's Last Name (MLN) is a field that is not linked between the Delivery Record and the Hospital Birth Record. They should be. The Date of Birth for the Mother of Child is linked which is great and if the MLN were linked it would save much time and effort especially when the MLN is particularly long and difficult to spell. The person entering the Delivery Record already has to type the MLN so linkage with the Birth Record would mean information already typed in would be displayed. This would save time and increase the accuracy of the data.

We have encountered our third person that has been diagnosed with a hand/wrist problem involving AVSS. The odds are bad since I have had only four people in that position since AVSS began. One person was diagnosed with Carpal Tunnel Syndrome, one starting to get Carpal and one diagnosed two weeks ago with Quervain's Synovitis. Two required physical therapy (and loss of time on the job).
If it could be possible to modify AVSS to be capable of printing the date of registration (field #17) at the same time the LFN# is printed, this would solve half of our problem. The other half is not yet solvable because we still have to put the health officer's signature on the certificate and initial.
Our occupational health doctor for Ventura County and Risk Management feels that if the U.S.A. can put a man on the moon, we should be able to circumvent hand stamping on a birth certificate. Those were his words, not ours. We told him we would try to solve one half of the problem.
We do know that the scanning would have to be done the same day as the date and LFN # being printed on the certificate.
Since this is a small office (three people) we really can't afford having people at the physical therapist's office on a ongoing basis.
P. S. (6/28/99) We have since found out we are allowed to type the Health Officer's Name in Field #16. This also would be of great help and more efficient time use to have this also entered during the LFN Assignment.

Problem/Suggestion: A problem was noted with running of the AVSS database integrity checker: after the checker is run the UNSENT index has a bunch of old records in it. I checked the ZFIXSENT routine and noted that it only sets the ISENT variable in each form.
The definition of the UNSENT index in ^FSU("FSX" uses the SENT node instead of the ISENT node to determine if the index should be set. The definition of the SENT index uses the ISENT node.
Since the SENT and UNSENT indexes are supposed to be compliments of each other they should be using the same variable.
Solution: I propose that we modify the UNSENT definition to use the ISENT node instead of the SENT. If we do this then the ZFIXSENT routine is okay and we won't have to define the SENT node in all the old forms.

Evaluation: The list of places of death is a standard list for all AVSS LRD sites. Consequently, a change that occurs at one site must be reflected at all other 55+ sites. The only realistic way to accomplish this goal is to use the current approach of making the change at the AVSS developmental site and distributing the list to all sites during each annual update. This is accomplished by submitting an AMR. If there is a need for quick response at a particular LRD, we suggest that the AMR be submitted by e-mail, and we will endeavor to make the update at the LRD on the day that it is received.

In Box #13 (Education), there is ample room, with no extra line needed.
In Box #124 (Describe how injury occurred), the software already provides two lines (124A and 124B).
In Boxes 14 (race), 40 (Place of final Disposition), 44 (Name of Funeral Director), 101 (Place of death), and 118 (Physician's name and address), there are times when two lines are needed. Without them the information would have to be incomplete This is especially true with Box #40 when the cremated remains go to the residence, which requires both name and address of the person receiving the remains, preceded by the abbreviation "RES". If the person has a hyphenated name, or lengthy address, it can require two lines of type. To make it "user friendly" you could provide, as a default, the address of the deceased, because most cremated remains are given to family at the same address as that of the deceased.
In Box #118, there are doctors who have objected to any variation of their name and address just to fit it on one line, and the mortuary needs to conform to the directions of the doctor.
1. In the draft death certificate registration software, provide for two lines of type for Boxes 14, 40, 44, 101, and 118 per state rule.
2. For Box #40, provide two separate areas, one for name, and one for address, with a default for the address giving that of the deceased as an option.

Problem/Suggestion: Over 1000 AVSS birth certificate records (LCAs) in 1998 had undefined values for mother's (and to a lesser extent, father's) state of birth (Field 10 and Field 7). Similar problems exist for father's state of birth and mother's state of residence. This results in an unacceptable number of unknowns in California's reporting to NCHS. Most of the values exist on the list of AVSS choices and the erroneous values are mainly the result of users not understanding how to select values from the AVSS list and the ease with which non-list values can be selected and from foreign systems. Examples of erroneous entries include misspellings such as AFGANISTAN, BANGLADAHS, COLUMBIA, GUATAMALA, etc. One frequent erroneous entry is AFRICA, a continent, not a country. Another problem is the use of punctuation such as CA., R.I., P.R., TX., etc. sometimes users enter a city such as BAGHDAD, DELHI, JERUSALEM, SAIGON, and TOKYO rather than a country. Occasionally users erroneously enter WITHHELD. A frequent example of not understanding how to use the AVSS list are variations on the District of Columbia as Wash DC, and many variants. Some parents still wish to use NB as the abbreviation for Nebraska, instead of the official NE. In some cases there are perhaps legitimate entries that should be added to the AVSS list, such as HOLLAND, KOSOVO, PALESTINE, SERBIA, and TAHITI. Nearly half of the erroneous entries originated in non-AVSS (foreign) computer systems.
Suggested Solutions: (for Fields 7 and 10)
1. Add HOLLAND, KOSOVO, PALESTINE, SERBIA, and TAHITI to the AVSS state/country list.
2. Disallow the entries of WITHHELD and any entries with punctuation.

Health Services, Office of Vital Records. The current procedure results in
the issuance of the Sealing Notice to the Local Health Department. The
local health dept., in turn, must locate the document, and perform the same
S&R that has already taken place at the OVR.
Solution: Set up a new reallocation system that is triggered at the State level by an
S&R action. Each S & R should set up the appropriate transfer global
reference to be used to reallocate the actual S&R to the appropriate County
Health Dept. This operation should work in conjunction with the current
reallocation sub system.
In conjunction with this new reallocation sub system, a special report
detailing the most recent updated list of sealed records could also be
created. The report (SRREPORT), should include Local File number, Name of
Child, Date of Birth, Date Reallocated, Date Sealed.

Problem/Suggestion: Due to creep in the current VS-10D (BC, rev5/97) forms, some hospitals had printer alignment problems using the HP FORM ALIGNMENT sub-option, since the maximum half line adjustment was insufficient. This sub-option used the routine FSALIGN with 21 different combinations of horizontal and vertical offsets. A different routine, FSHPLJ was developed to provide 'microadjust' capability of .02 inch increments by modifying a standard laser-jet peripheral file entry. This approach proved to be unreliable, however, and was used only in isolated sites at their request. In addition, this approach only allowed for one specific setting per site. The end result is two versions of AVSS printing at different sites, creating complications in updating and maintaining AVSS. A single, uniform approach to print alignment is therefore desirable.

Currently, Los Angeles is part of the 122 Cities Influenza-Pneumonia Surveillance System. In the past, selection of death records using underlying cause of death was sufficient to be included in the surveillance report. However, case selection criteria have now changed.
In order to be included into the tables for death by age, the use of the term "any mention of" has been incorporated into the case selection algorithm. Any mention of Influenza or Pneumonia, regardless of where it appears on the death record, is sufficient to count.
We do not code any mention of Influenza or Pneumonia. In addition, three other conditions are of public health importance, and could be useful in certain studies. These include Cancer, HIV, and Diabetes.
Solution: Add a new variable to the Death Record called AMO (any mention of). The only values that would be accepted would be I, P, C(cancer), H (HIV), or D (diabetes) and any combination of the five letters. The max length for the field would be five (5). Single letter values, as well as all combinations, would be acceptable responses.
For Example: Any Mention of Influenza would be coded as I. Any Mention of Pneumonia would be P. Any mention of both would be IP. Any mention of Cancer would be C. All five would be coded as IPCHD. Any mention of any combination of two, three, or four would also be valid.

Problem/Suggestion: In reviewing the CDC changes for EDRS, I came across an unnecessary global value. The LCA contains code in the %INIT field to do any special processing. However, the location of this node is in the SCH list. The CDC form uses the CHK list. I think the SCH list reference should be removed from the CDC form or we should set up the LCA form using the same methodology as the CDC form.

Problem/Suggestion: The #CMRDSEN report used IRACE to tabulate the race columns was in error in AVSS Version 4.8 and 4.9. Beginning in Version 4.8 there are two variables (RACE1 and ETHNICITY) in place of the original one (RACE). The two new ones had to be combined into the old one, and that was done OK. The problem is, there was a fourth variable (IRACE), composed of the first letter of RACE (as computed from RACE1 & ETHNICITY), and it was not updated with the Version 4.8 or 4.9.
Solution: Correct IRACE to combine RACE1 and ETHNICITY in the same was the two variables were combined into RACE. also run a correction program on all LRD AVSS databases to fix the IRACE variable for Version 4.8 and 4.9 records.

Problem/Suggestion: Because NCHS rules require preference to be given to Hawaiian (code 54) when it is reported with any other race, a new generated race code field will be included on the 2000 CBC record layout (888-889 for father and 981-982 for mother). If 54 appears in the 1st, 2nd or 3rd race code positions (001-006 for father and 007-012 for mother) the new generated race code field will contain 54. Otherwise, the new generated race code field will contain the code which appears in the 1st race code position (001-002 for father and 007-008 for mother).

Problem/Suggestion: I found a minor bug in the birth certificate form. The display coordinates for I18 and I18A are the same. The proper fix is to set the display coordinates for I18 to be 13,0 instead of 13,1. The globals that need to be updated are: ^FSF("HCA","I18") and ^FSF("LCA","I18"). After installation, the ^FSDI("HCA",0) display coordinate global should be killed off.

Problem/Suggestion: AMR 99-026, which allowed for up to 3 races entries for mother and father, reduced the length for Field 20C (Father's Education) and Field 23C (Mother's Education) on birth certificates from 13 to 7 characters. This will not allow the text 'WITHHELD' (a valid value) to be printed.
Solution: Abbreviate 'WITHHELD' as 'W/H' for Fields 20C/23C in the HCA and LCA forms. Code 'W/H" and 'UNK' as blank on the 1400 byte CBC record.

Problem/Suggestion: When entering in Field 7 (Father's State of Birth) or 10 (Mother's State of Birth) 'VI' for Virgin Islands, US, AVSS changes it to US VGN IS. If 'VIR' is entered, the user gets a list:

This also happens when a record is imported. If the record has 'VI' in Field 7 or 10 AVSS converts it to US VGN IS. When editing the record the only thing that will work is to type 'VIR' and get the list and select as in the example above.
